Upload Your Documents

Take photos of each of the documents below and upload them in the Dashboard.

  • Vehicle inspection form
Drivers in Colorado are required to have a vehicle inspection before becoming approved to drive. If you attend an onboarding event, your inspection will be performed there. If you visit a mechanic on your own instead, have them inspect your vehicle using this form.

  • Driver's license
Drivers are required to have a valid driver's license. A temporary license is acceptable, but be sure to upload your permanent copy when you receive it.
  • Personal vehicle insurance
Drivers are required to have personal vehicle insurance.
  • Driver photo
Lyft requires a photo of all drivers. Your driver photo is a passenger’s first impression of you when they request a ride.
  • Colorado TNC Medical Exam
Drivers in Colorado are required to have a TNC Medical Exam performed. If you attend an onboarding event, your medical exam will be performed there. If you visit a doctor on your own instead, follow these steps:
    1. Download and print the TNC Driver Medical Examination Packet.
    2. Bring the packet to a certified health professional (any MD, DO, PA or Nurse Practitioner) to have them complete it.
    3. Upload a clear image only of the last page of the packet (Medical Examiner's Certificate) in the dashboard.
    4. Keep the completed Medical Examiner's Certificate in your glovebox at all times while in driver mode.
  • Colorado vehicle registration
Drivers are required to upload a photo of their Colorado-issued vehicle registration.

Become Approved

Lyft will notify you when you've been approved as a driver. Until then, you can check your application status for updates.

Trade dress emblem placement
Drivers are required to display the Lyft emblem while in driver mode. The Lyft emblem is Lyft’s official trade dress. It helps passengers and law enforcement identify your vehicle.

The Lyft emblem should be displayed in the lower corner of your windshield on the passenger side every time you enter driver mode. Make sure to remove the emblem when you are not in driver mode.
